18 and pregnant with underage boyfriend.?

I recently found out that Im pregnant. My boyfriend is 15 and Im 18. Im not sure if I should say "unknown" as the father or put his name. I really don't want to get in trouble but I have to put something on all my welfare, wic, birth certificate, etc papers. Help?!?!
